Getting started
Create your first form
Sign in and click "New form" on the dashboard. You can either describe what you want in plain English (e.g. "A quote request form for a cleaning business") and let AI generate it, or start from a template in the gallery. Once generated, you'll see a live preview you can edit before publishing.
How AI form generation works
Type a sentence describing the form you need. The AI builds a complete form with relevant questions, choice options, and a scoring rubric. Free plans get 10 generations per month; Pro gets 300; Agency gets 1,000. You can regenerate individual blocks or the whole form without using a new generation credit.
Using a template
Browse the template gallery at /templates for 40 pre-built forms across categories like lead generation, job applications, feedback, quizzes, and registrations. Click "Use this template" to create an editable copy in your workspace. Templates are fully customizable — change questions, theme, logic, and scoring.
Forms & blocks
Available block types
FormHouse supports short text, long text, email, phone, number, URL, choice (single/multiple), dropdown, yes/no, opinion scale, rating, date, file upload, address, signature, payment, welcome screen, statement, and thank-you screen. Each block has configurable options like required, placeholder, and validation.
Conditional logic and branching
Add visibility rules to show or hide blocks based on previous answers. Add branching rules to skip to a different block based on conditions. Both use a simple when/then builder — no code needed. Logic is evaluated in real time as the respondent progresses.
Calculations and variables
Create variables to hold calculated values, hidden fields (populated from URL parameters), or constants. Use calculation rules to set or add to variables based on conditions. Reference variables in later questions using {{variable_name}} piping.
Publishing & sharing
Publish and share your form
Click "Publish" in the builder to make your form live. You'll get a shareable link at /f/your-form-slug. You can also embed it on your site using an iframe, or connect a custom domain via Cloudflare (Pro and Agency plans). Forms can be unpublished, closed, or deleted at any time.
Setting up a custom domain
Custom domains are available on Pro and Agency plans. Add your domain as a full DNS zone to your own free Cloudflare account, create a proxied A record pointing to the origin IP, and enable Flexible SSL. Then add the domain in FormHouse and complete TXT verification. FormHouse handles the Nginx routing automatically.
Customizing your form theme
Customize colors (background, question text, answer text, button), font family, button style (solid, outline, soft), and button radius. Upload a logo and choose its position. Add a background image or gradient with overlay opacity control. The builder preview shows changes in real time.
Inbox & scoring
How lead scoring works
When a form has scoring enabled with a rubric (a plain-English description of what makes a good lead), every submission is automatically scored 0–100 by AI. You'll see the score, a one-line rationale, and a short summary in your inbox. Free plans get 50 scored submissions/month; Pro gets 3,000; Agency gets 15,000.
Viewing and managing submissions
Open a form's inbox from the dashboard to see all submissions. Each row shows the score, email, and key answers. Click a submission to see the full response, score rationale, and summary. You can delete individual submissions or bulk-delete from the inbox.
Partial submissions
When partial submissions are enabled for a form, in-progress answers are saved automatically as the respondent types. If they abandon the form, you can still see what they entered before they left. Partials are auto-purged after 30 days. Available on Pro and Agency plans.
Understanding form analytics
Each form has an analytics page showing views, start rate, completion rate, drop-off by question, and submission trends over time. Use this to identify which questions cause abandonment and optimize your form for higher completion.
Billing & payments
Plans and pricing
Free includes unlimited forms and responses with FormHouse branding and limited AI. Pro is $29/month (or $279/year) with branding removal, one custom domain, and generous AI allowances. Agency is $79/month (or $759/year) with ten client workspaces and higher limits. The $10/month Booster adds 500 generations and 5,000 scored submissions.
Collecting payments in a form
Add a payment block to collect a deposit, booking fee, or full payment via PayPal. The amount can be fixed or calculated from previous answers using variables. FormHouse charges 0% platform fee — you only pay PayPal's standard transaction fee. Payments are available on Pro and Agency plans.
Canceling or changing your plan
Upgrade or downgrade between Pro and Agency at any time — changes take effect at the next billing cycle. Cancel your subscription to stop renewal; you keep paid access until the end of your current billing period. The AI Booster can be disabled immediately. All cancellations preserve your data — nothing is deleted.

Slack and webhook integrations
Add a Slack Incoming Webhook URL to get notifications in a Slack channel for each submission. Add a generic webhook to send signed POST requests to any URL. Webhook deliveries retry with backoff (30s, 2m, 10m, 30m, 2h, 6h) and can be toggled on/off per form. URLs must point to public addresses (private/internal IPs are blocked).
Importing from Typeform
Export your Typeform as a JSON file and import it at /import/typeform. FormHouse maps questions, choices, logic, and thank-you screens to the equivalent blocks. Review the imported form in the builder before publishing. Some advanced Typeform features may not have direct equivalents.
Spam & privacy
Spam protection features
Every form includes Turnstile (Cloudflare's CAPTCHA-free verification), honeypot fields, time-trap detection, and per-IP rate limiting — all enabled by default and included on every plan. Duplicate submission prevention is available via cookie-based controls. Suspected spam is silently accepted (not persisted) so attackers get no signal.
Data privacy and GDPR
FormHouse is hosted in the EU (Hetzner, Falkenstein) with encryption in transit and at rest. We never train AI models on your form content or submission data. Respondent data is processed under GDPR. You can delete submissions, forms, or your entire account at any time. See our Privacy Policy and GDPR pages for details.
